  3. Ex-Servicemen Contributory Health Scheme photo card.
  4. Pensioner card having photograph of the applicant.
  5. Certificate of identity in Original signed by a Gazetted officer.
  6. Bank certificate in Original on letter head from the branch(along with name and stamp of the issuing officer) containing duly attested photograph and bank account number of the applicant.
  7. Certificate of identity in Original signed by a Member of Legislative Assembly.
  8. Driving License.
  9. Arm’s license.
  10. Election Card / Voter ID.
  11. Certificate of identity in Original signed by a Member of Parliament.
  12. Central Government Health Scheme Card.
  13. Ration card having photograph of the applicant.
  14. Passport.
  15. Photo identity card issued by the Central Government or State Government or Public Sector Undertaking.
Address Proof (Any one):
  1. Latest property tax assessment order.
  2. Election Card / Voter ID.
  3. Domicile certificate issued by the Government.
  4. Driving License.
  5. Water Bill less than 3 months old.
  6. Certificate of identity in Original signed by a Member of Legislative Assembly.
  7. Aadhaar Card.
  8. Employer certificate in original.
  9. Passport of Spouse (Husband/Wife).
  10. Certificate of identity in Original signed by a Gazetted officer.
  11. Depository account statement less than 3 months old.
  12. Certificate of identity in Original signed by a Municipal Councilor.
  13. Electricity Bill less than 3 months old.
  14. Consumer gas connection card or book or piped gas bill.
  15. Landline/Broadband connection bill less than 3 months old.
  16. Passport.
  17. Allotment letter of accommodation issued by the Central/State Government less than 3 years old.
  18. Post office passbook having address of the applicant.
  19. Bank account statement less than 3 months old.

  10. Aadhaar Card.
  11. Affidavit sworn before a magistrate stating the date of birth.
  12. 10ᵗʰ or 12ᵗʰ Mark Sheet of recognized Board.
  13. Pension payment order.

Is PAN Card Reprint required?
Financial Institutions or Banks request a PAN Card as proof of PAN. If your PAN Card is stolen or misplaced, you can apply for PAN Card Reprint and present Reprint PAN Card.
Who has to sign the Reprint PAN Application?
The PAN Card Reprint Form must be signed by the PAN Holder. If he or she is unable to sign, a left thumb impression must be done on the PanReprint form.
